The Louisville Police Metro Department has finally released the incident report for the late Breonna Taylor, the 26-year-old Black woman who was shot by police who entered her home under a no-knock warrant, and it is primarily blank.

According to the Louisville Courier Journal, the four-page report lists the date, case number, time, incident location and Breonna’s full name, age and race. Her street number, apartment number and date of birth — all of which has been widely reported — were censored from the report.

Breonna’s injuries were listed as “none,” despite the fact that she was fatally shot at least eight times. The report also lists the charges as “death investigation — LMPD involved,” but checked the “no” box when asked did the incident occur from forced entry. The officers reportedly used a battering ram to knock in her apartment door.

The narrative portion of the report, where details of the incident could be written, only had two words: “PIU Investigation.”

The police department acknowledged errors in the report and released a statement about the inaccuracies in the report. “Inaccuracies in the report are unacceptable to us and we are taking immediate steps to correct the report and to ensure the accuracy of incident reports going forward,” the statement said.

Mayor Greg Fischer issued a statement Wednesday night (June 10) calling the released report “unacceptable.”

“Full stop. It’s issues like this that erode public confidence in LMPD’s ability to do its job, and that’s why I’ve ordered an external top-to-bottom review of the department,” he said. “I am sorry for the additional pain to the Taylor family and our community.”
